The purpose of this parental consent form is to obtain consent for your child’s photo image / livestream / video to be shared with Congregation Torat El and the local community. Photographs of students and livestream / video in the Religious School and our youth programs are routinely taken for the purpose of celebrating student achievement and sharing educational programs with the community. Photographs / livestream / video may be posted on the Congregation Torat El website, published in the Temple Bulletin, posted on the Temple Facebook / Instagram page, and/or submitted to area newspapers. The Student's Name will not be posted online. Consent for treatment in the event of a student medical emergency is also included. Authorization valid until rescinded in writing.

Medical Agreement in case of Medical Emergencies

I authorize the Director (or Director's designee) to seek appropriate medical care for my child, if necessary.
In case of a medical emergency requiring immediate emergency care, I authorize the paramedics to take my child to the nearest hospital if necessary. I understand that a reasonable attempt will be made to contact me in the event of a medical emergency, based on the nature and urgency of the situation.
It is agreed that I will hold Congregation Torat El harmless for the nature and outcome of any emergency medical treatment. It is also agreed that I leave the decision of what constitutes an emergency to the sole direction of the staff.
