School Registration Form 2026-2027

In case of emergency, we will call the following individuals in the order listed (include yourself if we should try and contact you first). Please do not list anyone that is difficult to reach by phone.

The purpose of this parental consent form is to obtain consent for your child’s photo image / livestream / video to be shared with Congregation Torat El and the local community. Photographs of students and livestream / video in the Religious School and our youth programs are routinely taken for the purpose of celebrating student achievement and sharing educational programs with the community. Photographs / livestream / video may be posted on the Congregation Torat El website, published in the Temple Bulletin, posted on the Temple Facebook / Instagram page, and/or submitted to area newspapers. The Student's Name will not be posted online. Consent for treatment in the event of a student medical emergency is also included. Authorization valid until rescinded in writing.

Thank you for paying CTE Religious School Tuition by Check. Please mail or deliver your check for the amount shown above to Congregation Torat El, 301 Monmouth Road, Oakhurst, NJ 07755. School Registration is complete once your payment is received.
